The website is designed to keep detailed clinical information out of the public appointment form and ordinary public contact channels.
The public form collects basic scheduling and contact information: patient/child name, parent or guardian name, mobile number, optional email address, appointment type, selected appointment slot and an optional short reason for the visit.
Do not submit detailed diagnoses, full clinical histories, ID numbers, payment-card details, medical reports, pathology documents or imaging through the public booking form or ordinary WhatsApp contact.
The booking form asks you to confirm that the practice may contact you about the appointment request using the telephone, WhatsApp or email details you provide.
When clinical records are required, the practice team will advise the appropriate process. The accounts email is for account queries and should not be used to send clinical documents.
Little Warriors Cancer Foundation is separate from the clinical practice. Foundation activities do not automatically receive or access information submitted to the medical practice.
