Clinical information: referral correspondence may contain sensitive health information. Referring organisations should use their approved secure email/document-transfer process, share only information necessary for the referral and confirm receipt when clinically important.
Referral checklist

- Brief clinical history and presenting concern
- Reason for referral and level of urgency
- Relevant laboratory, imaging and pathology results already available
- Current medication list and major treatment already given
- Referring clinician’s name, practice and direct contact details
